Sliding Fee Scale Discount Program

DFCC SLIDING FEE SCALE DISCOUNT
The DFCC Sliding Fee Scale Discount is based on current Federal Poverty Level Guidelines (FPL). SFSD is offered to all individuals without insurance based on countable income, family size and income percentage level of the FPL. The SFSD is based on the current DFCC visit rate. Additionally, the SFSD is offered to individuals with insurance who are unable to afford their co-pays. In the event that patients fall at or below 200% of the FPL and provide appropriate documentation, the staff will implement the DFCC Sliding Fee Scale Discount Program.
SLIDING FEE SCALE DISCOUNT PROGRAM
- On admission, The Clinical Office Associate or designee(s) will determine the insurance status of the patient.
- The Clinical Office Associate or designee(s) will provide the patient with information on health insurance programs offered by New York State for which they may be eligible.
- Patients with incomes that appear to qualify them for a NYS Insurance Program and demonstrate a willingness to file an application will be referred to a DFCC Certified Application Counselor (CAC) for assistance.
- Patients who present documented proof that they have applied for a New York State insurance program will not be billed and classified as Medicaid pending.

Cardiology

- Coronary Artery Disease, both treatment, and prevention. This includes chest pain-Angina, post-heart-attack care, and arrhythmias, including Atrial Fibrillation. In addition to our Primary Care providers, we have a Board-certified Cardiologist on our Staff;
- Hypertension/High Blood Pressure, the “silent killer.” We emphasize the impact of HBP on the risk of cardiovascular events, including heart attack, stroke, and kidney disease. Moreover, we now have a program that includes Remote Blood Pressure Monitoring, to help our patients monitor their blood pressure at home and have the data automatically sent to their provider; if elevated pressure is detected, the patient is advised to come in for medication adjustment;
- Valvular (Heart Valve) and Congenital Heart Disease: our Specialist is well versed in the diagnosis and treatment of these conditions.

Damian Family Care Centers is here to help – we offer MAT services. MAT can help you or your loved one live free of pain pills, heroin, and fentanyl. With MAT, your primary care provider has the option to give you a regular dose of a medication called buprenorphine/Suboxone that will help you manage the symptoms of withdrawal, like cravings, body aches, anxiety, and digestive issues. Many thousands of people have used MAT to overcome addiction and take control of their lives.
We offer MAT as part of comprehensive primary medical care, so you can get help with all of your primary medical needs in one place. People who need more intensive treatment for their substance use are referred to dedicated addiction treatment centers, while we continue to provide primary care services.

During the exam, the doctor will review your family and your medical history, determine any need for glasses, and conduct a check of the health of your eyes using eye drops if required. After the exam, the doctor will talk with you about your results and answer any questions you may have.
We offer a wide variety of vision services such as routine and comprehensive eye exams. We also offer general health exams to screen for eye diseases such as cataracts, glaucoma, floaters, and diabetic retinopathy. In addition, we prescribe eye medications for conditions such as dry eyes, allergies, and eye infections. We have a wide variety of glasses to choose from, for those with and without insurance. The pediatric service follows the American Academy of Pediatrics (AAP) guidelines on the periodicity of exams and utilizes age-specific health history templates to ensure age-appropriate care. Immunizations are administered according to AAP guidelines, focusing on achieving the US Public Health Service goal of 90% immunization rates in children by age two.
Our young patients are continually assessed for developmental delays during regularly scheduled well-child visits. Growth parameters are recorded and plotted on CDC standardized growth charts. Children found to have developmental delays or growth abnormalities are evaluated and referred accordingly. In addition, patients are screened for emotional and behavioral issues and are referred for mental health services if needed. Patients and their parents are provided with education surrounding the reason for the visit as well as anticipatory guidance during routine physical exam visits.
