Cardiology
Damian clinics offer expert care for the wide spectrum of heart diseases. Our providers are well-versed in the diagnosis and treatment of:
*Coronary Artery Disease, both treatment, and prevention. This includes chest pain-Angina, post-heart-attack care, and arrhythmias, including Atrial Fibrillation. In addition to our Primary Care providers, we have a Board-certified Cardiologist on our Staff;
*Hypertension/High Blood Pressure, the “silent killer.” We emphasize the impact of HBP on the risk of cardiovascular events, including heart attack, stroke, and kidney disease. Moreover, we now have a program that includes Remote Blood Pressure Monitoring, to help our patients monitor their blood pressure at home and have the data automatically sent to their provider; if elevated pressure is detected, the patient is advised to come in for medication adjustment;
*Valvular (Heart Valve) and Congenital Heart Disease: our Specialist is well versed in the diagnosis and treatment of these conditions.

Diabetes Care
We at the Damian Family Care Centers are committed to providing the best diabetes care to all of our patients. Our program is nationally recognized by the American Association of Diabetes Educators for providing quality diabetes care.
Our team includes:
Medical Providers: Our family doctors and nurse practitioners are dedicated to helping you manage your diabetes. They will keep your labs up to date, schedule yearly screenings like eye and foot exams, and ensure you’re on the correct medications.
Certified Diabetes Educators (CDEs): CDEs are licensed professionals who can help you learn about nutrition, medications, insulin, exercise, and many other ways of staying healthy.Ask your diabetes educator to show you how to use the “Healthy Plate Method” to manage your diabetes and lose weight.

Onsite A1C results: This important diabetes test is available in our own clinics, so you can get your results right away.
National Diabetes Prevention Program (NDPP) Groups: These groups are for patients that do not have diabetes but are at risk for developing diabetes. Risk factors for diabetes include:

Been diagnosed pre-diabetic by your doctor
NDPP is a lifestyle change program that consists of a series of sessions to help you make better food choices and increase your physical activity.
Gastroenterology: Ailments Of The Digestive System
Damian Providers are highly qualified to care for diseases of the GI system, including ailments as common as “indigestion” and Irritable Bowel Syndrome, as well as less common and more concerning conditions such as Liver disease/Hepatitis, Peptic Ulcer Disease, GERD (gastrointestinal reflux disease), GI bleeding, and cancers of the Colon, Pancreas, and Stomach. Our primary care providers are well-versed in these conditions, plus we have several experts in Gastroenterology to consult with if the condition is more complex or difficult to diagnose or treat.
*Women’s Healthcare
We provide quality healthcare services that meet the unique needs of women throughout their lives. Our family physicians, nurse practitioners, and our gynecologist, as well as our health educators, nutritionists, social workers, and behavioral health providers, work together to provide comprehensive and holistic care to women of all ages. We believe in helping women become healthy and stay healthy. Our staff spends time counseling patients on ways to promote health and well-being such as eating correctly, exercising, and getting specific tests and screenings based on a woman’s age and other factors. Our Director of HIV Care and Infectious Disease specialists also offer specialized care for women with HIV/AIDS.
Hepatology: Diseases Of The Liver
Damian providers are all Board-certified in their areas of expertise. We have several experts in diagnosing and treating the wide panoply of Liver diseases, including Hepatitis A, B, and C, both in their acute phases and in their chronic phases. These life-threatening conditions can often be controlled with specific anti-viral medications, which themselves have to be carefully monitored for side effects. Our experts are certified in Gastroenterology as well as Infectious Diseases. Our clinicians also help to ameliorate the ravages of chronic Cirrhosis due, most often, to alcohol abuse over a long period of time. We also diagnose and manage conditions related to pancreatitis and gall bladder disease.
HIV Primary and Specialty Care:
Damian provides caring, confidential diagnosis and treatment of HIV/AIDS in a private setting. Our goal is to empower people living with HIV/AIDS to maintain good health. We work with you as a team to coordinate your clinical services, case management, and health education so that you can stay well.

Medication-Assisted Therapy (MAT) For Opiod Addiction
Do you or a loved one struggle with pain pills, heroin, or fentanyl? Medications for Addiction Treatment (MAT) can help.
Each person has a personal path to recovery from opioid and other substance-use disorders (SUDs). Treatment with medication can help you or your loved one begin your recovery, regain your life and place in the community, and improve your relationships with family and friends. Damian Family Care Centers is here to help – we offer MAT services. MAT can help you or your loved one live free of pain pills, heroin, and fentanyl. With MAT, your primary care provider has the option to give you a regular dose of a medication called buprenorphine/Suboxone that will help you manage the symptoms of withdrawal, like cravings, body aches, anxiety, and digestive issues. Many thousands of people have used MAT to overcome addiction and take control of their lives. We offer MAT as part of comprehensive primary medical care, so you can get help with all of your primary medical needs in one place. People who need more intensive treatment for their substance use are referred to dedicated addiction treatment centers, while we continue to provide primary care services.

Behavioral health disorders involve a pattern of disruptive or destructive behavior that lasts several months or more. It isn’t a spur-of-the-moment action, but rather a pattern that has built up over time. Behavioral Health issues can include substance abuse, eating disorders, gambling addiction, and sex addiction. Each of these conditions stems from destructive behaviors. In children, behavioral health disorders may include ADHD or autism spectrum disorder.
We provide care in a comfortable, relaxing atmosphere so that families and individuals can find tranquility while focusing on their needs. Damian providers prescribe medication management and counseling for the treatment of anxiety, depression, substance use disorder, relationship concerns, as well as grief and bereavement support

Optometry At Damian
Poor vision severely affects performance at work, quality of life, and activities of daily living. If you tend to avoid driving at night, use over the counter reading glasses and drops, or get headaches related to eyestrain, then it is time to get your eyes checked. Many eye diseases and conditions often go undetected because they do not show any symptoms until they progress to an advanced stage. Whether you have had glasses for years or never had an eye exam before, the Optometry services at Damian Family Care Centers can help you see a bright future. Eye exams are usually easy and painless. During the exam, the doctor will review your family and your medical history, determine any need for glasses, and conduct a check of the health of your eyes using eye drops if required. After the exam, the doctor will talk with you about your results and answer any questions you may have.
We offer a wide variety of vision services such as routine and comprehensive eye exams. We also offer general health exams to screen for eye diseases such as cataracts, glaucoma, floaters, and diabetic retinopathy. In addition, we prescribe eye medications for conditions such as dry eyes, allergies, and eye infections. We have a wide variety of glasses to choose from, for those with and without insurance. The Optometry services at Damian Family Care Centers is committed to help you achieve, as well as maintain, clear and comfortable vision for your future.
Pediatric Services at Damian Family Care Centers
Health promotion and disease prevention are the foundations of pediatric health service delivery at Damian. Comprehensive health services, including evaluation and management of acute and chronic diseases, are provided to all children from newborn through adolescence. The pediatric service follows the American Academy of Pediatrics (AAP) guidelines on the periodicity of exams and utilizes age-specific health history templates to ensure age-appropriate care. Immunizations are administered according to AAP guidelines, focusing on achieving the US Public Health Service goal of 90% immunization rates in children by age two.
Our young patients are continually assessed for developmental delays during regularly scheduled well-child visits. Growth parameters are recorded and plotted on CDC standardized growth charts. Children found to have developmental delays or growth abnormalities are evaluated and referred accordingly. In addition, patients are screened for emotional and behavioral issues and are referred for mental health services if needed. Patients and their parents are provided with education surrounding the reason for the visit as well as anticipatory guidance during routine physical exam visits.

Pharmacy Assistance Program
If you can't afford your medications, Please ask to meet with one of our Case Managers to discuss if you qualify for assistance. Based on income levels and insurance status, you could qualify for various levels of assistance up to and including FULLY SUBSIDIZED Perscriptions.
Physical Therapy At Damian
Physical Therapy is often a key component of recovery from serious injuries, chronic pain, nerve damage, post-surgery, strokes, and motor vehicle accidents. Sports injuries also often require physical therapy before an athlete can return to the field of play. Regardless of your injury, Damian Family Care Centers offers the finest advanced physical therapy programs to help you recover as quickly and safely as possible.
At Damian, we offer outpatient physical therapy services for patients (adolescent to geriatric) recovering from orthopedic and neurological impairments of all types. After you have been examined by one of our highly trained specialists, we will customize a physical therapy program to meet your needs and goals. No two patients are exactly alike, and no two injuries are exactly alike. At Damian, we provide the individual attention required to devise a customized course of care that’s right for you.
